  1. Application for payment of a deceased employee's dues.

  2. Approved beneficiaries Form

  3. Declaration for non-drawing of pension from any govt. body

  4. Application for payment of additional grants.

  5. Copies of Identity documents of the beneficiaries and the legal representative.

  6. Letter indicating the person responsible for the funeral expenses in case of non-existence of widow or adult children. 

  7. Death certificate.

  8. Legal Notification for limitation of succession and legal power of attorney from the beneficiaries.
